About this Partnership

The California Chronic Care Coalition and the Healthcare Leadership Council are pleased to be founding members of this partnership to provide a grassroots consumer guide for adult immunizations and COVID-19. The objective of the Guide is to educate the public about novel treatments, medications, and vaccines for the SARS-CoV-2 and the importance of staying up to date on your immunizations. We come together to serve as a resource for anyone wishing to know more about adult vaccines, including the COVID-19 vaccine, that ensure the health and safety for all.

We envision a system of care that is accessible, affordable, and of a high-quality that emphasizes prevention, coordinated care, and the patient’s wellness and longevity, to include the early diagnosis of chronic conditions, access to effective and appropriate treatment and improved chronic care management.

Healthcare Leadership Council (HLC) is a coalition of chief executives from all disciplines within American healthcare, it is the exclusive forum for the nation’s healthcare leaders to jointly develop policies, plans, and programs to achieve their vision of a 21st century system that makes affordable, high-quality care accessible to all Americans.

Chief Medical Advisor: Dr. William Bommer

Dr. Bommer's service to the American College of Cardiology includes his current role on the California Government Relations Committee; as well as former roles as President, Vice-President, Governor, and Member of the Board of Governors. As an accomplished UC Davis Clinical Professor of Medicine, he directed the UC Davis Cardiology’s Noninvasive Services, the Cardiology Fellowship Training Program, and cares for both hospital inpatients and clinic outpatients. As the longest-tenured UCD Training Director, he has trained over 200 practicing cardiologists. Dr. Bommer has been the Principal Investigator or Co-Investigator of multiple NIH, NHLBI, and international research trials. He is a member of 50 international, national, state, and university education commissions and committees. Dr. Bommer founded and is a Board Member of several hardware and software startup companies and is an inventor of patents for xerography, color-flow ultrasound imaging, and contrast echocardiography. His public service includes quality improvement consulting with the State of California and he directs multiple programs including CA Pilot Percutaneous Coronary Intervention (PCI) (offsite) Program, CA Elective PCI (offsite) Program, and CA Cardiac Surgery and Intervention Outcomes Program. He has authored over 250 scientific publications, received over 100 Honors and Awards, presented over 1000 papers at International, National, and Statewide meetings, and coauthored California Legislation including SB 357 and SB 906. A Physics and Chemistry graduate of Cornell University, he received his medical degree from the State University of New York. He is an Honorary Lifetime Member of the British Cardiovascular Society and participates in running, biking, and water sports competitions.

Medical Advisor: Dr. Jose A. Arevalo

Dr. Jose A. Arevalo, MD has served as the Medical Director for Sutter Independent Physicians since 2003. Dr. Arevalo is a board-certified family physician with a long and prominent reputation as a teacher, researcher and administrator.

A graduate of Stanford Medical School, he completed his residency and clinical research fellowship in family medicine at University of California San Francisco. Dr. Arevalo has practiced medicine in numerous settings, including several community clinics and a multi-specialty medical group. He has also served on the faculty of the UC Davis Medical School for the last 21 years. As a full-time faculty member at UCD, his research focused on reproductive health, gestational diabetes, and Type 2 diabetes. His work led to numerous publications, the most notable, a 1988 Journal of the American Medical Association paper that helped revise national policy in screening for Hepatitis B in pregnancy. He was also the director of the Department's Medical Student Education Program and was named the 1994 U.C. Davis Kaiser Award for Excellence in Teaching. He was the first winner of the California Latino Medical Association Lifetime Achievement Award for Outstanding Contributions to Latino Health Issues in 1992. While at Davis, he was also Instructor-of-Record and primary physician consultant for Clinica Tepati, a student-run primary care clinic serving underserved patients in Sacramento's Alkali Flats, a role he proudly held for 16 years.

Dr. Arevalo has also been a consultant to the Department of Corporations and the lead physician for the Prenatal Outreach Unit for EDS, the Medi-Cal fiscal intermediary. As a member of the statewide Board of the American Diabetes Association California Affiliate, he worked with the Department of Health Service's Diabetes Control Program as one of the original members of the California Diabetes Steering Committee that created the Diabetes Coalition of California. As the first chair of the Guidelines Committee, he was the guiding force behind the Consensus Basic Diabetes Care Guidelines through CCHRI that are now widely accepted across California.

More recently he has been the Northern California regional medical director for Prudential Health Care, United Health Care and Blue Cross. Prior to his new role Dr. Arevalo was the Senior Vice President Medical Services for Health Plan of the Redwoods, a 78,000-member non-profit health plan in Sonoma County. Dr. Arevalo is the founding President and Chair of the Sacramento Latino Medical Association and serves on the Boards of the California Academy of Family Physicians and the Latino Coalition for a Healthy California. He is the past recipient of the California Wellness Foundation's Champions of Health Professions Diversity Award in 2006. He was also the recipient of the 2008 National Hispanic Health Foundation's First Leadership Award. He is married and is the proud father of seven and grandfather of three.
